Steps to enroll in services with Juniper Path

Step One

To start the process, you will need to fill out the online intake form found in the Contact dropdown above. If you choose to utilize insurance to pay for services, you will also need to provide:

  • A diagnostic report indicating F84.0 Autism Spectrum disorder as their diagnosis.

  • A referral for ABA therapy from your child’s doctor or psychologist.

  • Images of the front and back of your insurance card.

Step Three

Once all paperwork has been completed and your insurance has approved initiating the assessment, I will reach out to you to schedule meetings for initial assessment. These assessments take several hours of your time and include an initial clinical interview to learn more about your family, your child’s strengths and areas of growth, and your overall aspirations and hopes for what your child will get out of therapy. We will also take time to use this information to work together to create meaningful learning goals for caregivers. Typical assessments to expect:

  • Caregiver interview (1-1.5 hours)

  • Functional Behavior Assessments as needed (.5-1 hour)

  • Vineland Assessment caregiver form (45 min - 1 hour)

  • Direct observation of client (30 minutes)

  • Skills assessment (2-3 hours)

Step Two

Once your insurance coverage has been verified, I will reach out to schedule an initial meeting for us to get to know each other, review the caregiver handbook, and complete the rest of the required intake paperwork. Documents to review and sign before start of services:

  • Caregiver handbook

  • Notice of Privacy Practices

  • Consent for Release (signed consent is required for all outside provider collaboration)

  • Informed Consent to treat using ABA therapy

Step Four

When all assessments are complete, I will review data from all assessments and from any provided relevant documents. (diagnostic evaluation, referral, IEP, speech therapy treatment plan, etc.) I will create the initial treatment plan with specific goals for your child based on what we have decided are your top priorities and areas of need. When the initial treatment plan is completed, I will schedule a final intake meeting with you to review the treatment plan and ensure it aligns with your values, hopes and dreams, and areas of need you have identified for your child.

Once everyone is satisfied with goals, I will submit the initial treatment plan to your insurance for review. We will be able to schedule therapy and caregiver meetings once your insurance approves the proposed treatment plan. Most insurances typically take between 14-30 days to approve treatment.
